Media:Nameplates-template.svg is the name tag template. Open in a suitable program (CorelDraw, Inkscape, etc.) and edit.
I, Bry, have a bag of 100 on my member shelf. Feel free to grab one. If unavailable, the template fits standard 5/8 Slots for metal badge clips.
